Community Nursing
Our Community Nursing services are delivered by highly trained, AHPRA-registered nurses who place your health and wellbeing at the centre of care. We work with you, your family, and carers to design personalised care plans that reflect your needs, preferences, and goals. By collaborating with an interdisciplinary team of healthcare professionals, we ensure you receive comprehensive, safe, and flexible care that promotes dignity, independence, and quality of life.
Some of the nursing services will include the following:
- Continence Assessments and Management
- VAC Dressings/NTWT
- CVAD care and management – such as PICC or Midline line dressing
- Medication Management and Administration
- BGL Monitoring and Management
- End of Life / Palliative Care
- Urinary Catheter Management: IDC, Suprapubic (SPC), Uridome, Intermittent Catheter
- Ventilator Management
- Tracheostomy Management: Tracheostomy tube changing and managing/removal.
- Injections: Intramuscular, Subcutaneous, Intravenous, Intradermal
- Simple and Complex Wound Management: Burns, Acute and chronic Wound, Surgical,
- Trauma, pressure ulcers, skin tears among others
- Diabetes Management: Hypo and Hyper-glycaemic management, Diabetes assessment and education, BGL monitoring, Medication management – insulin and tablets, Emergency Management, lifestyle change etc.
- Working with an Interdisciplinary Team: E.g. Occupational Therapy, Speech Therapy, Dietician, Clinical Nurse Specialist, GPs, pharmacy, Specialists etc
- Assessment, monitoring and documentation of any and all the restrictive practices as per individual support plans in accordance to the NDIS guidelines.
